New patients, welcome!

Before Your Appointment

To speed up the initial process, download and fill out the following forms and bring them with you for your first appointment.

About our Facility

Lake Superior Dental Associates BuildingEnter through our main door from the parking lot and you will immediately see our entire office. To the left is our reception area: Relax before you are called in for your appointment. Our aquarium helps you relax—and entertains the children! Children may also visit our "nook" of books and toys while you enjoy one of our current magazines. However, we strive to not keep you waiting long, so you may not complete the article you start to read!

Your Appointment

Depending on the nature of your appointment, either a dental assistant or a dental hygienist will come and direct you to the dental operatory. There are two clinical floors—one above the reception/business office area and one below it. We have an open layout to provide easier movement for the doctors and staff as they deliver your care.

What to Expect

If you are new to our office and are not coming in for a dental emergency, you will first see a dentist for a complete, thorough oral examination to best assess your dental health for future scheduling. Depending upon the results of this exam, we accurately determine the time needed with the hygienist for your teeth cleaning at your next visit—unless we need to first schedule care for other dental concerns.

Financial

We accept major credit cards and personal checks. We offer a 5 percent discount for those who pay for services the day they are rendered. We also offer "Care Credit," an interest-free bank plan.

Dental insurance

We will gladly help you process your dental insurance. Consider contacting your Employee Benefit Administrator prior to your visit so you have your current information, plan coverage limits and deductibles. We're also familiar with most dental plans.
